Medicare Beneficiary Notice

This post serves as the Medicare Beneficiary Notice to our Medicare patients regarding participation in an Accountable Care Organization.  Originally posted 2021. Reposted on 9/3/2022.

Medicare requires our practice to notify our Medicare patients that we are participating in an Accountability Care Organization model (ACO).  We are also participants in Maryland's landmark MDPCP program.  The combined approach of these models with usual care (fee-for-service) have the overall impact of improving care, supporting the multi-disciplinary team we are building to meet our Medicare patients' healthcare needs, and growing as a practice. 

In 2007, CMS director, Dr. Donald Berwick, coined a term called "Triple Aim" which means that we work to improve patient satisfaction, lower the cost of healthcare, and improve outcomes.  In 2014, "Triple Aim" became "Quadruple Aim", adding clinician (and staff) satisfaction to the equation.  Then, in 2019, the term "Quintuple Aim" emerged, adding health equity as a priority of this type of work.

We believe that ACO participation helps our practice, our patients, and our team to thrive.  Your experience of ACO should be seamless.  That said, I am willing to discuss any questions you may have about this model.  In addition, our team is always open to the type of feedback which identifies any need for and ultimately promotes practice improvement.
